Description
We are seeking a skilled Physical Therapist (PT) to join our Outpatient Rehabilitation team in Menomonee Falls, Wisconsin. As a Physical Therapist, you will play a crucial role in providing high-quality patient care and helping individuals recover from injuries or illnesses.
The successful candidate will be responsible for interpreting physician referrals, completing evaluations, establishing plans of care, and monitoring patient outcomes. You will work closely with patients, families, and healthcare professionals to ensure seamless care coordination.
Key responsibilities include:
- Conducting musculoskeletal, neuromuscular, cardiopulmonary, and integumentary evaluations
- Developing and implementing individualized treatment plans
- Monitoring patient progress and adjusting treatment plans as needed
- Collaborating with physicians and other healthcare professionals
- Providing patient education and training
- Maintaining accurate records and documentation
Requirements:
- Physical Therapist license issued by the state of Wisconsin
- Basic Life Support (BLS) certification
- Bachelor's Degree in Physical Therapy
- Intermediate computer skills
- Excellent communication and interpersonal skills
