Job Summary: As a Case Management Coordinator, you will be responsible for assessing, planning, implementing, and coordinating case management activities with members to evaluate their medical needs and facilitate their overall wellness. You will develop proactive courses of action to address issues, enhance short-term and long-term outcomes, and identify opportunities to improve members' overall wellness.
Key Responsibilities:
- Conduct assessments of members' needs and benefit plan eligibility
- Facilitate integrative functions and smooth transitions to Aetna programs and plans
- Apply clinical judgment to incorporate strategies that reduce risk factors and barriers
- Address complex health and social indicators that impact care planning and resolution of member issues
- Review prior claims to address potential impact on current case management and eligibility
- Assess members' level of work capacity and related restrictions/limitations
- Utilize case management processes in compliance with regulatory and company policies and procedures
- Consult with supervisors and others to overcome barriers in meeting goals and objectives
Requirements:
- Minimum of 2 years of experience in case management, behavioral health, social services, or a related field
- Valid driver's license, preferably in IL
- Willing and able to travel using your own vehicle 50-75% of the time
- Bachelor's degree or non-licensed master-level clinician required, preferably with a degree in behavioral health or human services
Preferred Qualifications:
- Long-term care experience
- Case management or discharge planning experience
- Managed care experience
- Microsoft Office (Excel) experience

Salary Range: $21.10 - $44.99 per hour
