Physical Therapist

We are seeking a Physical Therapist to join our team at Advocate Health. As a Physical Therapist, you will interpret physician referrals and complete evaluations using appropriate methods. You will establish a plan of care, implement individual treatment, monitor and evaluate outcomes, and adjust the treatment plan as needed.

Responsibilities:

• Interpret physician referrals and complete evaluations
• Establish a plan of care and implement individual treatment
• Monitor and evaluate outcomes and adjust the treatment plan as needed
• Identify factors affecting patient's physical and psychosocial comfort and attend to these needs
• Plan for and complete patient discharge, including providing written home programs and patient/family teaching
• Communicate and collaborate with stakeholders to achieve optimum care for the patient
• Complete all required documentation and maintain site/department established productivity standards
• Participate in performance improvement activities and pursue professional growth through continuing education courses

Requirements:

• Doctorate degree in Physical Therapy from an accredited school
• Physical Therapy License in the State of Illinois or State of Wisconsin
• Basic Life Support (BLS) for Healthcare Providers (obtained within 90 days)
• Intermediate computer skills and excellent communication, interpersonal, and organization skills
