Physical Therapist - Cedartown Outpatient

We are seeking a skilled Physical Therapist to join our team at Advocate Health's Cedartown Outpatient location.

As a Physical Therapist, you will be responsible for providing comprehensive patient care, including assessments, treatments, and education. You will work closely with our interdisciplinary team to ensure high-quality patient outcomes.

Responsibilities:

• Provide patient care assessment and treatment, ensuring comprehensive and functional evaluations and treatments according to the Standard of Practice
• Develop and implement treatment plans with realistic goals, and encourage patients and significant others in setting goals
• Maintain accurate documentation, completing written patient evaluations, progress notes, and discharge notes per policy
• Meet department financial and productivity standards, adjusting individual schedules as needed
• Participate in process management to optimize quality and efficiency of services
• Promote professional development and education of other healthcare workers
• Communicate effectively with patients, physicians, staff, and external customers

Requirements:

• B.S., M.S., or D.P.T degree from an accredited physical therapy school
• Current licensure in the state of Georgia
• CPR certification in Basic Life Support, Level C
• Previous hospital experience preferred

• Contact with patients under various circumstances
