Physical Therapist

We are seeking a full-time Physical Therapist to join our team at Advocate Sherman Hospital - Acute Care Rehabilitation.

The Physical Therapist will interpret physician referrals, complete evaluations, establish plans of care, and implement individual treatments based on evaluation results and functional goals.

Responsibilities:

• Conduct musculoskeletal, neuromuscular, cardiopulmonary, and integumentary evaluations
• Develop and implement individualized treatment plans
• Monitor and evaluate patient outcomes, adjusting treatment plans as needed
• Provide patient/family teaching, referral for community services, and transition planning
• Complete required documentation, including patient evaluations, progress notes, and discharge summaries
• Participate in performance improvement activities and maintain productivity standards

Requirements:

• Doctorate degree in Physical Therapy from an accredited school
• Licensed Physical Therapist in the state of practice
• Basic Life Support (BLS) certification
• Intermediate computer skills and excellent communication, interpersonal, and organization skills
• Ability to lift, stand, walk, and squat throughout the workday

Preferred qualifications:

• Previous acute care experience
• Experience with EPIC EMR

Schedule details:

• Full-time position, 5 days/week (Monday - Friday)
• 2 evening shifts per week until 7:30 p.m. (Mondays and Thursdays)
• 1 weekend day shift every 4 weeks
• 1 summer and 1 winter holiday or weekend around a holiday

Compensation:

• $41.10 - $61.65 per hour
• $5000 sign-on bonus for full-time hires
