Physical Therapist

We are seeking a full-time Physical Therapist to join our team at AMG Lake Villa. As a Physical Therapist, you will interpret physician referrals and complete evaluations using appropriate methods. You will establish a plan of care, implement individual treatment, monitor outcomes, and adjust the treatment plan as needed. Responsibilities include:

• Interpreting physician referrals and completing evaluations
• Establishing a plan of care and implementing individual treatment
• Monitoring and evaluating outcomes and adjusting the treatment plan
• Identifying factors affecting patient comfort and attending to these needs
• Planning for and completing patient discharge
• Communicating and collaborating with stakeholders
• Completing required documentation and maintaining productivity standards
• Participating in performance improvement activities and pursuing professional growth

Requirements:

• Doctorate degree in Physical Therapy from an accredited school
• Licensed Physical Therapist in the state
• Basic Life Support (BLS) certification
• 1-2 years of experience in Pelvic Health or Lymphedema
• Intermediate computer skills and excellent communication and organization skills

$40.30 - $60.45 per hour. Sign-on bonus: $5,000 payable in two installments.
