Physical Therapist

We are seeking a full-time Physical Therapist to join our rehabilitation services team at AMG Lake Villa.

As a Physical Therapist, you will interpret physician referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, and integumentary evaluations. You will establish a plan of care, implement individual treatment, monitor and evaluate outcomes, and adjust the treatment plan as needed.

Responsibilities:

• Interpret physician referrals and complete evaluations
• Establish a plan of care and implement individual treatment
• Monitor and evaluate outcomes and adjust the treatment plan
• Identify factors affecting patient's physical and psychosocial comfort
• Plan for and complete patient discharge
• Communicate and collaborate with stakeholders
• Complete required documentation and maintain productivity standards
• Participate in performance improvement activities
• Pursue professional growth through continuing education

Requirements:

• Doctorate degree in Physical Therapy from an accredited school
• Licensed Physical Therapist in the state
• Basic Life Support (BLS) certification
• Intermediate computer skills
• Excellent communication, interpersonal, and organization skills

Physical Requirements:

• Ability to sit, stand, walk, lift, and squat throughout the workday
• Lift up to 50 lbs. from floor to waist
• Lift up to 10 lbs. from waist to overhead
• Push/pull with 30 lbs. of force
• Perform a sliding transfer of 150 lbs. with a second person present
