Physical Therapist (PT) - Outpatient

We are seeking a Physical Therapist (PT) for our outpatient clinic located in Stephenson, MI. The successful candidate will work full-time, with a schedule of 40 hours per week, and will be responsible for interpreting physician referrals, completing evaluations, establishing plans of care, and implementing individual treatments. The therapist will also monitor and evaluate patient outcomes, adjust treatment plans as needed, and communicate with physicians and other healthcare professionals.

Responsibilities:

• Interpret physician referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations
• Establish plans of care and implement individual treatments based on evaluation results and functional goals
• Monitor and evaluate patient outcomes, adjusting treatment plans as needed
• Communicate and collaborate with physicians, patients, and other healthcare professionals
• Complete required documentation, including patient evaluations, progress notes, and discharge summaries
• Participate in performance improvement activities and maintain productivity standards

Requirements:

• Doctorate degree in Physical Therapy from an accredited school
• Licensed Physical Therapist in the state where you work
• Basic Life Support (BLS) certification
• Intermediate computer skills and excellent communication, interpersonal, and organization skills

Salary and Benefits:

• $41.10 - $61.65 per hour
