Physical Therapist

We are seeking a full-time Physical Therapist to join our team at Advocate Health Care in Wisconsin.

The Physical Therapist will work in our outpatient rehabilitation clinic, interpreting physician referrals and completing musculoskeletal, neuromuscular, cardiopulmonary, and integumentary evaluations.

Responsibilities:

• Develop and implement individualized treatment plans based on evaluation results and functional goals
• Monitor and evaluate patient outcomes, adjusting treatment plans as needed
• Provide patient/family teaching, referral for community services, and ordering equipment
• Complete required documentation, including patient progress notes and discharge summaries
• Participate in performance improvement activities and manage assigned caseload
• Pursue professional growth through continuing education and department inservices

Requirements:

• Physical Therapist license issued by the state of Wisconsin
• Basic Life Support (BLS) for Healthcare Providers certification
• Bachelor's Degree in Physical Therapy
• Intermediate computer skills and excellent communication and interpersonal skills

Preferred Qualifications:

• Experience working in an outpatient rehabilitation setting

Work Schedule:

• 36 hours per week, including a combination of morning and evening hours
• Monday through Thursday: 6:30 AM - 6:00 PM, Friday: 6:30 AM - 5:00 PM

Salary: $41.10 - $61.65 per hour

Sign-on bonus: $5000 for full-time new hires
