Physical Therapist (PT) - Outpatient

We are seeking a skilled Physical Therapist (PT) to join our outpatient team at Advocate Health in Marinette, WI. As a Physical Therapist, you will play a crucial role in providing high-quality patient care, evaluating patients, developing treatment plans, and monitoring progress.

Responsibilities:

• Interpret physician referrals and complete evaluations using appropriate methods
• Establish a plan of care and implement individual treatment based on evaluation results and functional goals
• Monitor and evaluate outcomes, adjusting the treatment plan as needed
• Provide written home programs, patient/family teaching, and referral for community services
• Communicate and collaborate with stakeholders to achieve optimum care for patients
• Complete all required documentation and maintain site/department established productivity standards
• Participate in performance improvement activities and pursue professional growth through continuing education courses

Requirements:

• Doctorate degree in Physical Therapy from an accredited school
• Licensed Physical Therapist in the state of practice
• Basic Life Support (BLS) for Healthcare Providers certification
• Intermediate computer skills and excellent communication, interpersonal, and organization skills
• Ability to lift up to 50 lbs., stand, walk, and sit throughout the workday

Preferred Qualifications:

• Pediatric Physical Therapist preferred
• Experience working in a rural outpatient clinic
