Physical Therapist

We are seeking a full-time Physical Therapist to join our rehabilitation services team at Advocate Medical Group in Cicero, IL.

As a Physical Therapist, you will interpret physician referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, and integumentary evaluations. You will establish a plan of care, implement individual treatment, monitor and evaluate outcomes, and adjust the treatment plan as needed.

Responsibilities:

• Evaluate patients and develop plans of care
• Implement individual treatment plans based on evaluation results and functional goals
• Monitor and evaluate patient outcomes and adjust treatment plans accordingly
• Provide patient/family teaching, referral for community services, and transition planning
• Complete required documentation, including patient evaluations, progress notes, and discharge summaries
• Maintain site/department established productivity standards
• Supervise physical therapy assistants and extenders as needed

Requirements:

• Doctorate degree in Physical Therapy from an accredited school
• Licensed Physical Therapist in the state of Illinois
• Basic Life Support (BLS) certification
• Intermediate computer skills
• Excellent communication, interpersonal, and organization skills

Physical Requirements:

• Ability to sit, stand, walk, lift, and squat throughout the workday
• Lift up to 50 lbs. from floor to waist, up to 10 lbs. from waist to overhead, and carry up to 40 lbs. at waist height
• Push/pull with 30 lbs. of force and perform a sliding transfer of 150 lbs. with a second person present
