Home Care Physical Therapist

We are seeking a full-time Home Care Physical Therapist to join our team in University City, North Carolina.

The successful candidate will be responsible for managing a caseload of patients, ensuring timely care and documentation, and collaborating with multidisciplinary teams to provide high-quality patient outcomes.

Responsibilities:

• Manage daily patient caseload, ensuring timely care and documentation
• Collect and analyze patient data to identify high-risk patients and develop effective care plans
• Prioritize patient needs and coordinate care with multidisciplinary teams
• Evaluate patients to determine rehabilitation needs and develop plans of care
• Implement plans of care, perform physical therapy services, and direct care coordination
• Maintain communication with teams, physicians, and patients
• Incorporate short- and long-term goals into therapy and document progress

Requirements:

• Doctor of Physical Therapy (DPT) degree required
• Physical Therapy License in the state of practice required
• Basic Life Support for Healthcare Provider certification required
• Current state driver's license and auto insurance required
• Personal vehicle must be well-maintained and in safe driving condition

Preferred Requirements:

• 1-year experience as a licensed PT in a clinical care setting or home health preferred
• Pediatric experience within the past 5 years preferred
