Physical Therapist

We are seeking a full-time Physical Therapist to join our team at Advocate Condell Medical Center - Acute Care Rehabilitation.

As a Physical Therapist, you will interpret physician referrals and complete evaluations using appropriate methods. You will establish a plan of care, implement individual treatment, monitor and evaluate outcomes, and adjust the treatment plan as needed. You will also communicate and collaborate with stakeholders to achieve optimum care for patients.

Responsibilities:

• Interpret physician referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, and integumentary evaluations
• Establish a plan of care and implement individual treatment based on evaluation results and functional goals
• Monitor and evaluate outcomes and adjust the treatment plan as needed
• Communicate and collaborate with stakeholders to achieve optimum care for patients
• Complete all required documentation following department policy and accreditation standards
• Participate in performance improvement activities and pursue professional growth through continuing education courses

Requirements:

• Doctorate degree in Physical Therapy from an accredited school
• Physical Therapy License in the State of Illinois or State of Wisconsin
• Basic Life Support (BLS) for Healthcare Providers certification
• Intermediate computer skills and excellent communication, interpersonal, and organization skills
