Physical Therapist (PT) - Weekend Program- Acute Care

We are seeking a Physical Therapist (PT) for our weekend program in acute care at Aurora Medical Center Manitowoc County. As a Physical Therapist, you will interpret physician referrals, complete evaluations, establish plans of care, and monitor patient outcomes. You will work a schedule of 5 days a week, including Friday, Saturday, and Sunday, with weekend premiums applying.

Responsibilities:

• Interpret physician referrals and complete evaluations
• Establish plans of care and implement individual treatment
• Monitor and evaluate patient outcomes and adjust treatment plans
• Plan for patient discharge, including providing home programs and ordering equipment
• Complete required documentation and maintain accurate records
• Participate in performance improvement activities and pursue professional growth

Requirements:

• Physical Therapist license issued by the state of Wisconsin
• Basic Life Support (BLS) certification
• Bachelor's Degree in Physical Therapy
• Intermediate computer skills and excellent communication and interpersonal skills
