Description
Priory Hospital Middleton St George in Darlington, County Durham, has an opportunity for a full-time Social Worker. The hospital provides specialist inpatient services for adults with mental health issues, complex needs, and challenging behaviours.
The Social Worker will be an active member of the multidisciplinary team, participating in ward rounds, risk assessments, care planning, discharge planning, decision-making, and team meetings. The successful candidate will play a key part in patient recovery, supporting the assessment and delivery of tailored treatment plans while ensuring dignity, respect, and high standards of care.
Key responsibilities include:
- Engaging effectively with service users to assist with their rehabilitation and recovery
- Providing a link with service users' locality services and the multidisciplinary team
- Providing a link with service users' social supports and the multidisciplinary team
- Acting as Carer Champions for the hospital and promoting carer engagement in service users' treatment
- Supporting and encouraging visits, ensuring they are supportive and beneficial to service users
- Ensuring child visits are in the best interest of the child and properly risk-assessed
- Identifying, assessing, and contributing to the management of risk
- Acting as a lead in multi-agency contact relating to safeguarding adults, children, and young people
- Identifying and challenging discriminatory structures and practices
The ideal candidate will have a degree in Social Work, be registered with Social Work England, and have completed the ASYE or equivalent post-qualifying experience. They should have experience managing a caseload independently, undertaking assessments, risk management, safeguarding enquiries, and care planning, as well as working with multi-agency partners.
