Physical Therapist - Outpatient Rehabilitation at Home

We are seeking a Physical Therapist to join our new at-home program serving outpatient therapy across the Northern Charlotte area.

As a Physical Therapist with Advocate Health, you will deliver high-quality, one-on-one outpatient physical therapy services in patients' homes, helping improve mobility, function, and independence.

Responsibilities:

• Evaluate patients and develop individualized treatment plans
• Provide evidence-based outpatient care, collaborating with physicians, caregivers, and interdisciplinary team members
• Utilize technology and flexible scheduling to manage a caseload across a defined geographic area
• Document evaluations, progress notes, and discharge notes as needed
• Communicate patient progress to physician, referral sources, and other clinical staff

Requirements:

• Bachelor's degree in Physical Therapy program required
• Applicable state license for Physical Therapy required
• BLS required per policy guidelines
• Ability to lift patients, equipment, or supplies up to 20 pounds frequently and 50 pounds occasionally
