Physical Therapist- Advocate Trinity Outpatient

We are seeking a Physical Therapist to join our team at Advocate Trinity Hospital - Outpatient Rehabilitation. As a Physical Therapist, you will interpret physician referrals and complete evaluations using appropriate methods. You will establish a plan of care, implement individual treatment, monitor outcomes, and adjust the treatment plan as needed. Responsibilities include:

• Conducting musculoskeletal, neuromuscular, cardiopulmonary, and integumentary evaluations
• Developing and implementing individualized treatment plans
• Monitoring and evaluating patient outcomes
• Collaborating with physicians and other healthcare professionals
• Providing patient/family teaching and discharge planning
• Maintaining accurate records and completing required documentation
• Participating in performance improvement activities and quality improvement initiatives

Requirements:

• Doctorate degree in Physical Therapy from an accredited school
• Licensed Physical Therapist in the state where you work
• Basic Life Support (BLS) certification
• Intermediate computer skills
• Excellent communication, interpersonal, and organizational skills

Preferred qualifications:

• Experience working in an outpatient rehabilitation setting
• Certification in a specialty area of physical therapy
