Outpatient Physical Therapist - Pool B

We are seeking an Outpatient Physical Therapist to join our team at Advocate Illinois Masonic Medical Center. As an Outpatient Physical Therapist, you will interpret physician referrals and complete evaluations using appropriate methods. You will establish a plan of care, implement individual treatment, monitor and evaluate outcomes, and adjust the treatment plan as needed.

Responsibilities:

• Interpret physician referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations
• Establish a plan of care and implement individual treatment based on evaluation results and functional goals
• Monitor and evaluate outcomes and adjust the treatment plan as needed
• Provide status updates to physicians and other teammates
• Identify factors affecting patient's physical and psychosocial comfort and attend to these needs
• Plan for and complete patient discharge, including providing written home programs, patient/family teaching, and referral for community services
• Communicate and collaborate with stakeholders to achieve optimum care for the patient
• Complete all required documentation, including patient evaluations, progress notes, and discharge summaries
• Maintain site/department established productivity standards
• Understand and adhere to all billing and documentation policies, procedures, and timelines

Requirements:

• Doctorate degree in Physical Therapy from an accredited school
• Licensed Physical Therapist in the state where you work
• Basic Life Support (BLS) for Healthcare Providers certification
• Intermediate computer skills
• Excellent communication, interpersonal, and organization skills
• Demonstrated organization, decision-making, and problem-solving skills

Physical Requirements and Working Conditions:

• Ability to sit, stand, walk, lift, and squat throughout the workday
• Lift up to 50 lbs. from floor to waist
• Lift up to 10 lbs. from waist to overhead
• Lift and carry up to 40 lbs. at waist height a reasonable distance
• Push/pull with 30 lbs. of force
• Perform a sliding transfer of 150 lbs. with a second person present
