Physical Therapist- Outpatient

We are seeking a Physical Therapist to join our outpatient team at Advocate Health in Oswego, IL. As a Physical Therapist, you will interpret physician referrals and complete musculoskeletal evaluations. You will establish a plan of care, implement individual treatment, monitor outcomes, and adjust treatment plans accordingly.

Responsibilities:

• Interpret physician referrals and complete evaluations
• Establish a plan of care and implement individual treatment
• Monitor and evaluate outcomes, adjusting treatment plans as needed
• Provide patient/family teaching, referral for community services, and transition planning
• Complete required documentation and maintain productivity standards
• Participate in performance improvement activities and pursue professional growth

Requirements:

• Doctorate degree in Physical Therapy from an accredited school
• Licensed Physical Therapist in the state
• Basic Life Support (BLS) certification
• Intermediate computer skills and excellent communication, interpersonal, and organization skills

Preferred Qualifications:

• Experience in upper extremities
• Certified hand therapist

Salary: $41.10 - $61.65 per hour
