Physical Therapist, Float, Outpatient

We are seeking a Physical Therapist, Float, Outpatient to join our team at Advocate Health. As a Physical Therapist, you will provide high-quality patient care, evaluate patients, and develop treatment plans to enhance physical and functional abilities.

Responsibilities:

• Provide age-appropriate patient care, demonstrating knowledge of growth and development principles across the lifespan
• Evaluate patients, interpret medical records, and establish patient care plans using evidence-based interventions
• Set functional goals with patients and caregivers, monitor outcomes, and adjust treatment plans as needed
• Educate patients and caregivers on diagnosis, treatment, and home instructions
• Communicate effectively with healthcare providers, staff, and external customers
• Complete required documentation, maintain productivity standards, and adhere to billing and documentation policies
• Participate in performance improvement initiatives, professional development, and continuing education
• Meet requirements for the Enterprise Float Program, demonstrating competence across assigned departments and locations

Requirements:

• Degree in Physical Therapy from an accredited institution
• Doctor of Physical Therapy (DPT) preferred or commensurate educational requirements
• Physical Therapy License in the state(s) of employment
• Basic Life Support (BLS) certification for Healthcare Providers
• Intermediate computer skills and excellent communication, interpersonal, and organizational skills
