Description
We are seeking a part-time Physical Therapist (PT) to join our Outpatient Rehabilitation team in Waterford, WI. As a Physical Therapist, you will play a crucial role in providing high-quality patient care, focusing on orthopedic and musculoskeletal diagnoses, including conservative and post-operative care for shoulder, knee, hip, spine conditions, tendinopathies, stroke rehab, balance impairments, and generalized weakness.
Responsibilities:
- Interpret physician referrals and complete evaluations using appropriate methods.
- Establish and implement individualized treatment plans based on evaluation results and functional goals.
- Monitor and evaluate patient outcomes, adjusting treatment plans as needed.
- Plan for patient discharge, including providing written home programs and referring for community services.
- Complete required documentation and maintain accurate records.
- Collaborate with healthcare professionals to achieve optimum patient care.
- Participate in performance improvement activities and pursue professional growth through continuing education.
Requirements:
- Doctorate degree in Physical Therapy from an accredited school.
- Licensed Physical Therapist in the state of practice.
- Basic Life Support (BLS) certification.
- Intermediate computer skills and excellent communication, interpersonal, and organizational skills.
- Ability to lift up to 50 lbs., stand, walk, and sit throughout the workday.
