Description
We are seeking an Advanced Nurse Practitioner to join our multidisciplinary primary care team at Law Medical Group Practice in London. The successful candidate will deliver high-quality, patient-centred clinical care, supporting the assessment, diagnosis, and management of a wide range of acute and long-term conditions.
The Advanced Nurse Practitioner will work autonomously within their scope of practice, contributing to clinical decision-making, prescribing, and care planning. The post holder will also play an active role in supporting the wider multidisciplinary team and contributing to the ongoing development and objectives of the practice.
Key responsibilities include:
- Managing acute and chronic conditions
- Conducting health assessments
- Running specialist clinics
- Prescribing medications
- Promoting health and disease prevention
- Contributing to clinical policy development
- Supporting safeguarding
- Participating in clinical audits and service improvement
The practice operates across two sites: Willesden (main site) and Wembley (branch site). The practice has a list of 18,000 patients and is supported by a team of healthcare professionals, including GPs, physician associates, practice nurses, and phlebotomists.
The ideal candidate will have:
- A postgraduate diploma or degree in Advanced Practice
- Qualified Nurse Prescriber status
- Experience working in a primary care environment
- Experience in chronic disease management and prescribing
