Physical Therapist - Home Health

We are seeking a Physical Therapist to join our Home Health team in the Far Southwest Suburbs, focusing on Midlothian, Crestwood, Robbins, Blue Island, Calumet Park, and Alsip. As a Physical Therapist, you will provide high-quality patient care, facilitate quality improvement initiatives, and collaborate with the healthcare team to achieve optimal health outcomes.

Responsibilities:

• Assess patients' physical needs and develop plans of care
• Provide skilled physical therapy interventions, including exercises and education
• Collaborate with patients, families, and healthcare teams to achieve goals
• Participate in quality improvement initiatives and adhere to organisational policies
• Maintain accurate records and documentation

Requirements:

• Licensed Physical Therapist in the State of Illinois (or eligible for licensure)
• 1 year of clinical experience or strong clinical affiliation experience
• Basic computer skills, including Microsoft Word and email
• Ability to communicate professionally and work effectively with patients and healthcare teams
• Strong time management and organisational skills

Schedule:

• Monday - Friday, 8am - 5pm
• Rotating weekends (every 5 weeks) and holidays (1 summer and 1 winter holiday per year)
