Description
We are seeking a Physical Therapist (PT) to join our Inpatient team at Advocate Health. As a Physical Therapist, you will play a crucial role in the rehabilitation of patients, working closely with our healthcare team to deliver high-quality care.
Job Summary: The Physical Therapist (PT) - Inpatient will interpret physician referrals, complete musculoskeletal, neuromuscular, cardiopulmonary, and integumentary evaluations, and establish plans of care for patients. The successful candidate will implement individualised treatment plans, monitor patient progress, and adjust plans as needed. They will also participate in patient discharge planning, provide education to patients and families, and communicate effectively with healthcare teams.
Responsibilities:
- Interpret physician referrals and complete evaluations using appropriate methods
- Establish plans of care and implement individualised treatment based on evaluation results and functional goals
- Monitor and evaluate patient outcomes, adjusting treatment plans as needed
- Plan for and complete patient discharge, including providing written home programs and patient/family teaching
- Communicate and collaborate with stakeholders to achieve optimum patient care
- Complete required documentation, including patient evaluations, progress notes, and discharge summaries
- Maintain site/department established productivity standards and adhere to billing and documentation policies
Requirements:
- Doctorate degree in Physical Therapy from an accredited school
- Licensed Physical Therapist in the state where you work
- Basic Life Support (BLS) certification
- Intermediate computer skills and excellent communication, interpersonal, and organisation skills
Physical Requirements and Working Conditions:
- Ability to sit, stand, walk, lift, and squat throughout the workday
- Lift up to 50 lbs. from floor to waist, up to 10 lbs. from waist to overhead, and carry up to 40 lbs. at waist height
- Push/pull with 30 lbs. of force and perform a sliding transfer of 150 lbs. with a second person present
