Physical Therapist - Home Health Virtual Team

We are seeking a Physical Therapist to join our Home Health Virtual Team. As a Physical Therapist, you will provide video consultation and triage to patients, families, and significant others according to the Home Health Virtual Program plan of care.

Responsibilities:

• Provide ongoing care via video according to the established plan of care
• Determine priorities of patient care based on essential patient needs and available resources
• Recognize changes in the patient's status, need for care, and intervene appropriately
• Coordinate patient interventions such as physician contacts, consultation with patient's home health care team, and facilitation of appropriate visit strings
• Assess patient, family, and/or caregiver's training needs and provide teaching/counseling video consultation/triage
• Collect and maintain clinical and patient records according to agency, state, and federal guidelines
• Assist in completion of reports and statistics related to key clinical performance measures
• Ensure quality of care through development, implementation, and evaluation of professional standards

Requirements:

• Physical Therapist license issued by the state in which the teammate practices
• Bachelor of Science, Masters, or Doctorate Degree in Physical Therapy
• Typically requires 5 years of experience in a combination of rehab, home care, and triage settings
• Strong communication skills, detail-oriented, and ability to coordinate and manage complex care for patients over video
• Advanced computer skills using Microsoft Office applications
