Physical Therapist (PT) - Outpatient

We are seeking a full-time Physical Therapist to join our Outpatient Rehabilitation team at Aurora Medical Center Oshkosh: Fond du Lac Campus. As a Physical Therapist, you will interpret physician referrals, complete evaluations, establish plans of care, and implement individual treatment based on evaluation results and functional goals. You will also monitor and evaluate outcomes, adjust treatment plans, and communicate with physicians and other teammates.

Responsibilities:

• Interpret physician referrals and complete evaluations
• Establish plans of care and implement individual treatment
• Monitor and evaluate outcomes and adjust treatment plans
• Communicate and collaborate with stakeholders to achieve optimum care
• Complete required documentation and maintain productivity standards
• Participate in performance improvement activities and pursue professional growth

Requirements:

• Doctorate degree in Physical Therapy from an accredited school
• Licensed Physical Therapist in the state of practice
• Basic Life Support (BLS) certification
• Intermediate computer skills and excellent communication, interpersonal, and organization skills

Preferred Requirements:

• Experience working with pediatric and orthopedic populations
