Physical Therapist

Join our innovative virtual team supporting patients across the state of Wisconsin as a Physical Therapist. Treatments are completed on site at 9200 Loomis Road in Franklin within our dedicated physical department space. This is a full-time position to treat patients virtually with occasional requirements to treat patients on site at nearby clinics.

Major Responsibilities:

• Interpret physician referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations.
• Establish a plan of care and implement individual treatment based on evaluation results and functional goals.
• Monitor and evaluate outcomes and adjust the individualized treatment plan based on the patient's response to treatment.
• Plan for and complete patient discharge, including providing written home programs and patient/family teaching.
• Complete all required documentation, including patient progress notes and discharge summaries.
• Clean up after patient treatment and maintain a neat and orderly workspace.
• Demonstrate skill in use of modalities, exercise equipment, and other devices.
• Participate in performance improvement activities and manage assigned caseload.
• Pursue professional growth through continuing education courses and department inservices.

Licensure, Registration, and/or Certification Required:

• Basic Life Support (BLS) for Healthcare Providers certification.
• Physical Therapist license issued by the state in which the team member practices (Wisconsin or Illinois).

Education Required:

• Bachelor's Degree in Physical Therapy.

Experience Required:

• No experience required.

• Intermediate computer skills.
• Excellent communication and interpersonal skills.
• Demonstrated organization, decision-making, and problem-solving skills.

Physical Requirements and Working Conditions:

• Must be able to sit, stand, walk, lift, and squat throughout the workday.
• Lift up to 50 lbs. from floor to waist, up to 10 lbs. from waist to overhead, and carry up to 40 lbs. at waist height.
• Push/pull with 30 lbs. of force and perform a sliding transfer of 150 lbs. with a second person present.
