Description

Jewish Care is looking for an experienced Social Worker to join our Social Work and Community Support service. You will provide professional social work support to adults across North East London, Central London, Essex, and Southend, supporting people to maintain their independence, improve their wellbeing, and remain safely within their communities.

You will manage a complex caseload of approximately 35 cases (FTE), carrying out holistic assessments, developing person-centred support plans, and delivering effective social work interventions. You will work with people experiencing a range of challenges, including ageing, disability, mental health needs, dementia, social isolation, end-of-life care, and other complex or co-existing needs.

You will use your professional judgement to assess risk, respond to referrals, identify safeguarding concerns, and ensure people receive the right support at the right time. The role also involves providing advice, information, and advocacy, helping individuals and carers navigate health and social care systems and access appropriate services.

Partnership working will be an important part of the role. You will work closely with colleagues across Jewish Care and build effective relationships with statutory, voluntary, and community organisations. You will also represent Jewish Care at external meetings and forums and contribute to the development of community-based initiatives, support groups, and other activities that promote social inclusion and wellbeing.
