Physical Therapist MW (PT) - Outpatient - Pelvic Health and Vestibular

We are seeking a part-time Physical Therapist to join our Outpatient Rehabilitation team at Advocate Condell Medical Center. As a Physical Therapist, you will interpret physician referrals and complete evaluations using appropriate methods. You will establish a plan of care, implement individual treatment, monitor outcomes, and adjust the treatment plan as needed. Responsibilities include:

• Conducting musculoskeletal, neuromuscular, cardiopulmonary, and integumentary evaluations
• Developing and implementing individualized treatment plans
• Monitoring and evaluating patient outcomes
• Collaborating with physicians and other healthcare professionals
• Providing patient/family teaching and discharge planning
• Maintaining accurate records and completing required documentation
• Participating in performance improvement activities

Requirements:

• Doctorate degree in Physical Therapy from an accredited school
• Physical Therapy License in Illinois or Wisconsin
• Basic Life Support (BLS) certification
• Intermediate computer skills
• Excellent communication, interpersonal, and organization skills

Preferred Qualifications:

• Experience in Pelvic Health and Vestibular rehabilitation

Work Schedule:

• Part-time, 20 hours per week
• Monday - Friday, with two evenings until 7:00 pm

Salary Range:

• $40.30 - $60.45
