Physical Therapist (PT) - Home Health Case Manager

We are seeking a Physical Therapist (PT) - Home Health Case Manager to join our team at Advocate Health. As a Physical Therapist, you will play a crucial role in ensuring patients receive quality, cost-effective care in the comfort of their own homes.

Responsibilities:

• Assess patients' physical needs and develop personalized plans of care
• Collaborate with patients, families, and healthcare teams to achieve optimal health outcomes
• Utilize evidence-based practices and stay up-to-date with the latest standards in physical therapy
• Participate in quality improvement initiatives and promote patient safety
• Communicate effectively with patients, families, and healthcare professionals
• Prioritize and organize patient care, ensuring clear hand-off communication
• Initiate referrals and discharge planning for assigned patients

Requirements:

• Physical Therapy License in the State of Illinois
• Basic Life Support (BLS) for Healthcare Providers certification
• Valid driver's license
• Doctorate degree in Physical Therapy from an accredited school
• 1 year of clinical experience or strong spectrum of clinical affiliation experience
• Basic knowledge of computer use and ability to learn computer-based scheduling and documentation systems
• Strong communication and interpersonal skills

Schedule:

• Full-time position, 40 hours per week
• Monday - Friday, 8am-5pm, with rotation to weekends and 1 summer/winter holiday
