Physical Therapist (PT) - Outpatient

We are seeking a Physical Therapist (PT) to join our Outpatient Rehabilitation team at Advocate Health in Marinette, WI. This is a full-time position with a competitive salary range of $41.10 - $61.65 per hour.

The successful candidate will be responsible for interpreting physician referrals, completing evaluations, establishing plans of care, and implementing individual treatment plans. They will also monitor and evaluate patient outcomes, adjust treatment plans as needed, and provide status updates to physicians.

Responsibilities:

• Interpret physician referrals and complete evaluations
• Establish plans of care and implement individual treatment plans
• Monitor and evaluate patient outcomes and adjust treatment plans as needed
• Provide status updates to physicians and attend to patient needs
• Plan for and complete patient discharge, including providing written home programs and patient/family teaching
• Complete required documentation and maintain accurate records
• Participate in performance improvement activities and pursue professional growth

Requirements:

• Physical Therapist license issued by the state of Wisconsin
• Basic Life Support (BLS) certification issued by the American Heart Association
• Bachelor's Degree in Physical Therapy
• Intermediate computer skills and excellent communication and interpersonal skills
• Ability to lift up to 50 lbs. and push/pull with 30 lbs. of force
