Description
We are seeking an Advanced Nurse Practitioner to join our team at Amersham Vale Practice in Deptford, Lewisham. The successful candidate will be part of a team of 2 other ANPs and will play a key role in managing our duty desk and triage service.
The role involves assessing online requests, prioritizing clinical need, and deciding on the safest and most appropriate care for each patient. This may include remote or face-to-face management, signposting to other services, or escalation to urgent or secondary care.
The post holder will provide clinical care within a team consisting of general practitioners, advanced healthcare professionals, practice nurses, and healthcare assistants. Patient contact will be mainly via triage, on-call duties, face-to-face clinics, and home visits.
Key responsibilities include:
- Carrying out clinical nursing practice at an advanced level using expert clinical skills, critical judgments, and knowledge to deliver appropriate and safe care
- Triage and treat patients, performing specialist assessment of their needs and making referrals as necessary to other members of the primary and secondary healthcare teams
- Prescribing and reviewing medication that is appropriate to patient needs and in accordance with latest evidence-based practice, national and practice protocols, and within the scope of qualified practice
- Diagnosing and managing acute and chronic conditions, including drug and non-drug-based treatment methods into a management plan
- Requesting diagnostic investigations and interpreting within agreed protocols
- Assessing, diagnosing, and treating patients in practice premises or their own home who require medical attention and/or referring to appropriate agencies

The salary range for this role is £37-£40 per hour, depending on experience. The contract is permanent, and the working pattern is full-time or part-time.
