Description
Intermountain Health is seeking a Lifestyle Medicine Physician to join their team at the Live Well Center in Park City, UT. As a Physician in Lifestyle Medicine, you will work with a team of healthcare professionals committed to providing quality care and an excellent patient experience. In this role, you will provide clinical services to patients for preventing, treating, and managing chronic diseases and improving quality of life.
The position includes a $15,000 sign-on bonus and up to $20,000 in relocation assistance. You will practice at the Intermountain Health Live Well Center in Park City, UT. Experienced and new graduate physicians with expertise in Lifestyle Medicine, Obesity Medicine, Integrative Medicine, or Functional Medicine will be considered.
Requirements:
- MD or DO degree from a fully accredited allopathic or osteopathic school of medicine
- Residency training and board certification/eligibility in Family Medicine, Internal Medicine, OB/GYN, Physical Medicine & Rehabilitation or other applicable ABMS specialty
- Board Certification/Eligibility or fellowship training in Lifestyle Medicine, Obesity Medicine, Functional Medicine, or Integrative Medicine
- Active, unobstructed Utah medical license, or the ability to obtain one
- Full prescriptive authority and an active DEA, or the ability to obtain one
- Ability to successfully complete Intermountain Health's credentialing process

The hourly range for this position is $7.25 - $999.99. The scheduled weekly hours are 40.
