Description

The Inclusion Health Outreach Nurse will be at the heart of an exciting and innovative approach to improving health outcomes for people experiencing rough sleeping, homelessness, multiple disadvantage and severe social exclusion in Hounslow.

Through flexible, trauma-informed healthcare support, the postholder will help reach individuals who often face significant barriers to accessing mainstream services.

The service has been developed from a collaboration between Hounslow Council and Little Park Surgery.

The postholder will be employed by Little Park Surgery, an innovative NHS GP surgery that is a leader in developing services to tackle health inequity.

Clinical governance and support for this role will be provided by the GPs working at Little Park Surgery.

Main duties of the job

  • Provide first point of clinical contact for patients presenting with a variety of immediate and necessary needs
  • Undertake holistic health assessments for individuals experiencing rough sleeping and homelessness
  • Deliver flexible outreach-based healthcare interventions, in a variety of community locations
  • Provide wound care, infection management, physical health monitoring and health screening
  • Undertake health observations, blood testing and basic clinical investigations
  • Request and chair multi-disciplinary meetings

Job responsibilities

The role offers a unique combination of street-based outreach, clinical assessment, treatment, health promotion, care coordination and system navigation.

The postholder will support individuals with complex physical health needs, mental health challenges, neurodiversity, substance misuse issues and safeguarding concerns, delivering care in a way that is accessible, responsive and tailored to individual circumstances.

A key part of the role will be building trusted and meaningful relationships with people who may have experienced repeated trauma, stigma and exclusion from services.

By engaging individuals earlier and supporting them to navigate health and care systems, the postholder will play a vital role in preventing deterioration, improving health outcomes, increasing engagement with services and reducing avoidable hospital admissions.
