Physical Therapist - Inpatient Rehab Per Diem

We are seeking a Physical Therapist to join our Inpatient Rehab team at Advocate Lutheran General Hospital. As a Physical Therapist, you will play a crucial role in the rehabilitation of patients, working closely with our multidisciplinary team to develop and implement individualized treatment plans.

Responsibilities:

• Interpret physician referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, and integumentary evaluations
• Establish a plan of care and implement individual treatment based on evaluation results and functional goals
• Monitor and evaluate outcomes, adjusting the treatment plan as needed
• Communicate and collaborate with stakeholders to achieve optimum care for patients
• Complete required documentation, including patient evaluations, progress notes, and discharge summaries
• Participate in performance improvement activities and pursue professional growth through continuing education

Requirements:

• Doctorate degree in Physical Therapy from an accredited school
• Licensed Physical Therapist in the state of Illinois
• Basic Life Support (BLS) certification
• Intermediate computer skills and excellent communication, interpersonal, and organization skills

Physical Requirements:

• Ability to sit, stand, walk, lift, and squat throughout the workday
• Lift up to 50 lbs. from floor to waist, up to 10 lbs. from waist to overhead, and carry up to 40 lbs. at waist height
• Push/pull with 30 lbs. of force and perform a sliding transfer of 150 lbs. with a second person present
