Physical Therapist

We are seeking a full-time Physical Therapist to join our outpatient rehabilitation team at the Fond du Lac Campus. As a Physical Therapist, you will interpret physician referrals, complete evaluations, establish plans of care, and implement individual treatment based on evaluation results and functional goals. You will also monitor and evaluate outcomes, adjust treatment plans, and communicate with physicians and other teammates.

• Communicate and collaborate with stakeholders
• Complete required documentation and maintain productivity standards
• Participate in performance improvement activities and pursue professional growth

Requirements:

• Doctorate degree in Physical Therapy from an accredited school
• Licensed Physical Therapist in the state
• Basic Life Support (BLS) certification
• Intermediate computer skills and excellent communication, interpersonal, and organization skills
