Physical Therapist

We are seeking a Physical Therapist to join our team at Advocate Health. As a Physical Therapist, you will be responsible for the daily management of assigned patient caseload, ensuring timely care and documentation to meet patient needs.

Responsibilities:

• Collect and analyze data to determine patient issues and available resources
• Prioritize multi-issue situations based on patient health and welfare
• Evaluate patients to determine rehabilitation needs and develop plans of care
• Establish and implement plans of care, performing physical therapy services and directing care coordination
• Maintain communication with multidisciplinary teams and provide updates on patient conditions
• Incorporate short and long-term goals into therapy and document progress

Requirements:

• Doctor of Physical Therapy (DPT) degree required
• Physical Therapy License in the state of practice required
• Basic Life Support for Healthcare Provider certification required
• Current state driver's license and auto insurance required
• Demonstrates knowledge of principles of growth and development and skills appropriate to patient ages and needs

Preferred Requirements:

• 1-year experience as a licensed PT in a clinical care setting or home health preferred
• Pediatric setting – 1 year pediatric experience within the past 5 years preferred
