Physical Therapist

We are seeking a part-time Physical Therapist to join our team at Advocate Trinity Hospital - Acute Care Rehabilitation.

The Physical Therapist will interpret physician referrals, complete evaluations, and establish plans of care for patients. They will also monitor and evaluate patient outcomes, adjust treatment plans as needed, and communicate with physicians and other healthcare professionals.

Responsibilities:

• Interpret physician referrals and complete evaluations
• Establish plans of care and implement individual treatment based on evaluation results
• Monitor and evaluate patient outcomes and adjust treatment plans as needed
• Communicate and collaborate with physicians and other healthcare professionals
• Complete required documentation and maintain accurate records
• Participate in performance improvement activities and contribute to quality of care
• Supervise physical therapy assistants and students as needed

Requirements:

• Doctorate degree in Physical Therapy from an accredited school
• Licensed Physical Therapist in the state of practice
• Basic Life Support (BLS) certification
• Intermediate computer skills and excellent communication and organizational skills

Physical demands:

• Ability to sit, stand, walk, lift, and squat throughout the workday
• Lift up to 50 lbs. from floor to waist and up to 10 lbs. from waist to overhead
• Push/pull with 30 lbs. of force and perform a sliding transfer of 150 lbs. with a second person present
