Physical Therapist- Outpatient

We are seeking a Physical Therapist to join our outpatient team in Zion, IL. As a Physical Therapist, you will be responsible for evaluating and treating patients with physical disabilities, injuries, or illnesses. You will work with patients to develop and implement treatment plans, provide education on exercises and injury prevention, and communicate with healthcare providers.

Responsibilities:

• Evaluate patients and develop diagnosis and treatment plans
• Provide treatment and intervention to patients, including therapeutic exercises and modalities
• Develop and implement discharge plans
• Communicate with patients, families, and healthcare providers
• Document patient progress and treatment plans
• Participate in quality assurance and process improvement initiatives

Requirements:

• Doctorate in Physical Therapy
• Previous PT experience
• Physical Therapist licensure in Illinois
• Ability to lift up to 100 pounds and perform heavy lifting occasionally

Preferred Qualifications:

• Experience with pelvic health
• Certification in OCS or SCS
