Physical Therapist (PT) - Outpatient

We are seeking a part-time Physical Therapist (PT) to join our Outpatient Rehabilitation team at Advocate Health in Franklin, WI.

As a Physical Therapist, you will interpret physician referrals and complete evaluations using appropriate methods. You will establish a plan of care, implement individual treatment, monitor outcomes, and adjust the treatment plan as needed. Additionally, you will communicate with patients, families, and healthcare teams to ensure optimal care.

Responsibilities:

• Interpret physician referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, and integumentary evaluations
• Establish a plan of care and implement individual treatment based on evaluation results and functional goals
• Monitor and evaluate outcomes, adjusting the treatment plan as needed
• Communicate with patients, families, and healthcare teams to ensure optimal care
• Complete required documentation, including patient evaluations, progress notes, and discharge summaries
• Participate in performance improvement activities to identify and resolve problems affecting patient care outcomes

Requirements:

• Doctorate degree in Physical Therapy from an accredited school
• Licensed Physical Therapist in the state where you work
• Basic Life Support (BLS) for Healthcare Providers certification
• Intermediate computer skills and excellent communication, interpersonal, and organization skills

• $41.10 - $61.65 per hour
• 20 hours per week, part-time position
