Join Oak Street Health's NP Fellowship in Adult Primary Care, a 12-month paid fellowship that provides the necessary skills, support, and learning to become a highly confident and competent clinician for older adults.
The Role: Start your NP career in the future of healthcare! Oak Street Health's NP Fellowship in Adult Primary Care is in formal collaboration with the University of Michigan School of Nursing. The program is set in the revolutionary value-based care model, which rewards healthcare companies for the quality of their care, not the quantity.
During the program, fellows spend time building their own patient panel, learning from a multitude of preceptors, and engaging in didactic content that aims to build on, not repeat, graduate training.

Requirements:
- Family, adult, or adult-gerontology primary care nurse practitioners who graduated or will graduate from their programs between May 2025 and December 2026 or Physician Assistant who graduated or will graduate from their programs between May 2025 and November 2026
- Must currently reside in, or be willing to relocate to, the Fellowship program location for at least the next two years
- Have not yet worked as an NP or PA
- Minimum of one year experience, within the last five years, working as an adult RN or minimum one year experience in healthcare (for PAs)
- Minimum GPA of 3.0 in NP/PA program
- Have passed, or in the process of, passing NP/PA board examinations by 12/7/26
- Currently holds or will hold by 12/21/26 an active, non-probationary state NP/PA license
- A genuine passion for adult primary care
- A supportive attitude toward our patient population of older adults
