Physical Therapist (PT) - Outpatient

We are seeking a full-time Physical Therapist to join our Outpatient Rehabilitation team at Advocate Health's Aurora Medical Center Oshkosh: Fond du Lac Campus.

As a Physical Therapist, you will interpret physician referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, and integumentary evaluations. You will establish a plan of care, implement individual treatment, monitor and evaluate outcomes, and adjust the treatment plan as needed.

Responsibilities:

• Evaluate patients and develop plans of care
• Implement individual treatment plans and monitor patient progress
• Collaborate with physicians and other healthcare professionals
• Complete required documentation and maintain accurate records
• Participate in performance improvement activities and contribute to quality of care
• Supervise physical therapy assistants and students as needed

Requirements:

• Doctorate degree in Physical Therapy from an accredited school
• Licensed Physical Therapist in the state where you work
• Basic Life Support (BLS) certification
• Intermediate computer skills and excellent communication, interpersonal, and organization skills
