Physical Therapist (PT) - Outpatient

We are seeking a Physical Therapist (PT) for our Outpatient Rehabilitation department at Aurora Medical Center Manitowoc County. As a Physical Therapist, you will play a crucial role in providing high-quality patient care.

Job Summary: The Physical Therapist will interpret physician referrals, complete evaluations, and establish plans of care for patients. You will monitor patient outcomes, adjust treatment plans, and provide education to patients and families.

Responsibilities:

• Interpret physician referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, and integumentary evaluations
• Establish plans of care and implement individual treatment based on evaluation results and functional goals
• Monitor and evaluate patient outcomes, adjusting treatment plans as needed
• Provide patient/family education, discharge planning, and community resource referrals
• Complete required documentation, including patient progress notes and discharge summaries
• Participate in performance improvement activities and manage assigned caseload

Requirements:

• Physical Therapist license issued by the state of Wisconsin
• Basic Life Support (BLS) certification within 6 months
• Bachelor's Degree in Physical Therapy
• Intermediate computer skills and excellent communication and interpersonal skills

Working Conditions:

• Ability to sit, stand, walk, lift, and squat throughout the workday
• Lift up to 50 lbs. from floor to waist, up to 10 lbs. from waist to overhead, and carry up to 40 lbs. at waist height
• Push/pull with 30 lbs. of force and perform a sliding transfer of 150 lbs. with a second person present
