Description
We are seeking a Physical Therapist- Outpatient to join our team at Advocate Christ Medical Center - Outpatient Rehabilitation in Tinley Park, IL.
As a Physical Therapist, you will interpret physician referrals and complete evaluations using appropriate methods. You will establish a plan of care, implement individual treatment, monitor outcomes, and adjust the treatment plan as needed. Responsibilities also include communicating with patients, families, and healthcare teams, as well as providing education and training.
The ideal candidate will have a Doctorate degree in Physical Therapy from an accredited school and a license to practice in Illinois. Excellent communication, interpersonal, and organizational skills are essential.
Responsibilities:
- Interpret physician referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations
- Establish a plan of care and implement individual treatment based on evaluation results and functional goals
- Monitor and evaluate outcomes, adjusting the treatment plan as needed
- Communicate and collaborate with patients, families, and healthcare teams
- Provide education and training to patients and families
- Complete required documentation and maintain accurate records
Requirements:
- Doctorate degree in Physical Therapy from an accredited school
- Licensed Physical Therapist in Illinois
- Basic Life Support (BLS) certification
- Intermediate computer skills
- Excellent communication, interpersonal, and organizational skills
Physical Requirements and Working Conditions:
- Ability to sit, stand, walk, lift, and squat throughout the workday
- Lift up to 50 lbs. from floor to waist, up to 10 lbs. from waist to overhead, and carry up to 40 lbs. at waist height
- Push/pull with 30 lbs. of force and perform a sliding transfer of 150 lbs. with a second person present
