Physical Therapist (PT) - Home Health - South Sub West Suburbs

We are seeking a Physical Therapist (PT) to join our Home Health team in the South Sub West Suburbs. As a Physical Therapist, you will play a crucial role in providing high-quality patient care in a home health setting.

Responsibilities:

• Assess patients' physical needs and develop plans to help them recover from injuries or illnesses
• Provide therapy sessions to patients in their homes, focusing on improving mobility, strength, and function
• Collaborate with other healthcare professionals to ensure comprehensive care
• Educate patients and families on exercises, treatments, and self-care techniques
• Document patient progress and adjust treatment plans as needed

Requirements:

• Licensed Physical Therapist in the State of Illinois (or eligible for licensure)
• 1 year of clinical experience or strong spectrum of clinical affiliation experience
• Basic knowledge of computer use (Microsoft Word, email, etc.)
• Ability to learn computer-based scheduling and documentation systems
• Excellent communication and interpersonal skills
• Strong time management and self-organization skills
• Proficient use of home-based therapy equipment and assistive devices
