Physical Therapist

We are seeking a Physical Therapist to join our team at Advocate Health. As a Physical Therapist, you will interpret physician referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, and integumentary evaluations. You will establish a plan of care, implement individual treatment, monitor and evaluate outcomes, and adjust the treatment plan as needed.

Responsibilities:

• Interpret physician referrals and complete evaluations
• Establish a plan of care and implement individual treatment
• Monitor and evaluate outcomes and adjust the treatment plan
• Identify factors affecting patient's physical and psychosocial comfort
• Plan for and complete patient discharge
• Communicate and collaborate with stakeholders
• Complete required documentation and maintain productivity standards
• Participate in performance improvement activities
• Pursue professional growth through continuing education

Requirements:

• Doctorate degree in Physical Therapy from an accredited school
• Licensed Physical Therapist in the state
• Basic Life Support (BLS) certification
• Intermediate computer skills
• Excellent communication, interpersonal, and organization skills

Salary: $41.10 - $61.65 per hour
