Description
Humana/iCare is seeking an RN Care Manager to join the team in the Family Care Partnership (FCP) program to provide services to members in Milwaukee and surrounding counties.
The RN Care Manager will provide case management and support to adults in the community. Responsibilities include:
- Completing the nursing portion of the comprehensive assessment and providing input to the member's care accordingly
- Monitoring wellness and health promotion activities based on the member's assessment and identified outcomes
- Re-evaluating the member's health and wellness at appropriate intervals to manage ongoing health issues
- Performing routine health monitoring activities based on the member's need
- Building constructive working relationships with members, their family members, and outside agency staff
- Providing or arranging for services, supports, or medical care that best meet the member's identified needs
- Helping with program and procedure development and assisting with input for the QI/QA program
- Traveling to any location within Milwaukee and surrounding counties
Requirements:
- Must be a Registered Nurse, licensed in the State of Wisconsin in good standing
- Two (2) years of related health care experience
Preferred Qualifications:
- Bachelor of Science degree in Nursing (BSN)
- Previous home health, managed care, or case management experience
- General knowledge of Medicaid and Medicare benefits
Additional Information:
- Workstyle: Field - Work remotely from your home and in the field visiting members
- Travel: up to 50% throughout Milwaukee and surrounding counties
- Typical Work Days/Hours: Monday – Friday, 8:30 am – 5:00 pm Central Standard Time (CST)
Driving:
- Requires a valid state driver's license and proof of personal vehicle liability insurance with at least 100/300/100 limits
- Mileage reimbursement is provided for work-related travel
TB Screening:
- This role is considered patient-facing and is part of Humana's Tuberculosis (TB) screening program
Work at Home Requirements:
- Internet service must meet minimum criteria
- Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information
Scheduled Weekly Hours: 40
Pay Range: $71,100 - $97,800 per year
