Physical Therapist, Outpatient

We are seeking a Physical Therapist to join our team at Rural Critical Access Hospital in Sparta, NC. As a Physical Therapist, you will provide primary outpatient care to patients across the age spectrum, from adolescents to the elderly.

Responsibilities:

• Provide patient care assessment and treatment, ensuring comprehensive and functional evaluations and treatments according to the Standard of Practice
• Develop and implement treatment plans with realistic goals, and encourage patients and significant others in setting goals
• Maintain accurate documentation, completing written patient evaluations, progress notes, and discharge notes per policy
• Ensure quality patient outcomes, supervising support staff and providing direction and feedback
• Participate in process management to optimize quality and efficiency of services, identifying quality and operational opportunities
• Promote professional development and education of other healthcare workers, providing representation on committees and conducting in-service training programs
• Communicate effectively with patients, physicians, staff, and external customers, responding to questions and promoting an open atmosphere of communication

Requirements:

• B.S., M.S., or D.P.T degree from an accredited physical therapy school
• Current licensure in the state of North Carolina
• CPR certification in Basic Life Support, Level C
