Home Health Physical Therapist (PT) Case Manager - Marinette Service Market

We are seeking a Full-time Salaried Physical Therapist Case Manager for our Marinette patient service market in North Eastern WI.

The successful candidate will conduct physical therapy evaluations, establish plans of care, and provide ongoing assessments of patient responses to treatments.

Responsibilities:

• Conduct physical therapy evaluations and establish plans of care
• Provide ongoing assessments of patient responses to treatments and update plans of care accordingly
• Communicate effectively with physicians, management, and patients
• Manage assigned caseloads, including scheduling and ensuring adequate coverage
• Participate in chart audits and continuous quality improvement activities

Requirements:

• Physical Therapy License in the State of Illinois or Wisconsin
• Basic Life Support (BLS) for Healthcare Providers
• A valid driver's license
• Doctorate degree in Physical Therapy from an accredited school
• 1 year of clinical experience
