## Description

We are seeking a Physical Therapist to join our team on a floating basis between Advocate Oak Lawn and Advocate Trinity sites.

As a Physical Therapist, you will provide outpatient physical therapy services to patients of various ages and conditions.

**Major Responsibilities:**

- Provide age-appropriate patient care, demonstrating knowledge of growth and development principles across the lifespan

- Evaluate patients, develop care plans, and implement evidence-based interventions

- Establish functional goals with patients and caregivers, monitor outcomes, and adjust plans as needed

- Educate patients and caregivers on diagnoses, treatment plans, and home exercises

- Communicate effectively with healthcare providers, staff, and external customers

- Complete accurate and timely documentation, maintain productivity standards, and adhere to billing and compliance policies

- Participate in performance improvement initiatives, professional development, and continuing education

**Requirements:**

- Physical Therapy license in the state of employment

- Basic Life Support (BLS) certification for Healthcare Providers

- Doctor of Physical Therapy (DPT) preferred or equivalent educational requirements

- Intermediate computer skills and excellent communication, interpersonal, and organizational skills

**Physical Requirements and Working Conditions:**

- Ability to sit, stand, walk, lift, and squat throughout the workday

- Lift up to 50 lbs. from floor to waist, up to 10 lbs. from waist to overhead, and carry up to 40 lbs. at waist height

- Push/pull with 30 lbs. of force and perform a sliding of 150 lbs. with a second person present

- Repetitive use of hands for simple grasping, pushing/pulling, and fine manipulation
